Home and Community Care Access
Provides intake for home health and community care services for residents with acute, chronic, palliative or rehabilitative health care needs. Case managers provide assessments, evaluate the client needs, determine eligibility and provide recommendations for services to be offered.
Services include case management, home care nursing, respite, home support, palliative or hospice care, assisted living and residential care, adult day programs, and community rehabilitation.
