REACH Community Health Centre

Provides individuals and families connections to community resources to support their social and economic needs. Volunteers work with patients to connect them with the appropriate resources in many areas such as housing, income security, employment, food security, child development, and social services, in order to target underlying barriers to good health.

Provides weekly addictions support groups to men from the Latin American community. Also provides information and referral to other community members.

Works with immigrants and refugee communities to deliver culturally-specific health promotion programs. Offers a variety of health and wellness programs, health education presentations, parenting programs, and self-management and prevention groups.

Also provides assistance in navigating health and social services systems, computer drop-in, community garden program, and information on accessing services or adapting to living in Canada.

Offers a variety of support groups to help members with adjusting to the community. Groups are offered in a number of different languages and include culturally specific health education presentations, wellness programs, senior support groups, children's programs, and groups specifically for men and women.

Provides a full range of nutritional counselling services for individuals and families. Focuses on healthy foods and integrating healthy habits into one's lifestyle. Offers cooking and food access programs for clients with barriers to eating nutritious foods. Advocate for food security.

Provides team-based culturally sensitive primary care, pharmacy services, dental care for community members including Indigenous people, immigrants, and refugees in the Vancouver's Commercial Drive area.

Offers assistance and referrals to other community-based programs and services for those facing challenges related with shelter and housing, income security, employment, food security, child development assistance and social services.

Offers primary care (family doctor or nurse practitioner).

Provides a full range family practice and medical services provided by physicians, nurse practitioners, registered nurses, pharmacists, counselors and nutritionists. On-call services available after hours, weekends and on holidays.

Provides pharmacy services for individuals to obtain, understand, and manage their prescriptions. Offers a full range of prescriptions and over-the-counter medication; open to everyone - you do not need to be a REACH patient.
