Complex Care Program at BC Children's Hospital

Provides case planning and family support to children and youth with complex, chronic medical care needs such as genetic syndromes and neurologic impairment, frequent hospitalizations, multiple medications, and high intensity care needs.

The care team works with the child's local pediatrician or health care provider to coordinate care, as well as helping families with setting appointments, decision making and goal setting.
