Convalescent Care

Provides short-term care to patients who no longer need acute care in hospital, but do need more time to recover before returning home. A team of health care professionals work together to help patients increase strength and mobility so they can safely carry out activities of daily living when at home.

Eligibility

Targeted Population Restricted

Medically stable adults with chronic illness or disabilities. Clients must meet home health eligibility needs to receive services including citizenship and residency requirements.
