Home and Community Care Access
Fraser Health Access Line (previously called the Home Health Service Line)
Provides intake for home health and community care services for residents with acute, chronic, palliative or rehabilitative health care needs. Case managers provide assessments, evaluate the client needs, determine eligibility and provide recommendations for services to be offered.
Services include assisted living and residential care, rehabilitation services, home nursing, case management, home support, adult day services, hospice and palliative care, community nutrition, and respite.
