Palliative Care Access Lines

Provides telephone intake for access to palliative care services for people living with life-limiting illness and their families. Services are provided in the home, a hospice, hospital or care facility depending on the stage of illness and the client's needs.

Support services include nursing care, home support, personal care, pain management, and home hospice specialist services.
