Home and Community Care at Dog Creek Health Centre

Offers home-based services for adults with disabilities, chronic or acute illnesses, and the elderly. Services are culturally appropriate, accessible, and respond to the unique health and social need of the community.

Services include nursing assessments, treatments and procedures, home support, personal care, respite, hospital discharge planning and follow-up, health monitoring, transportation services, and access to medical supplies and equipment.

Targeted Population Restricted

The elderly, persons living with disabilities, the chronically or acutely ill requiring home-based treatment or care, and persons recently discharged from hospital requiring care.
Application Process
Self-referral, or referrals from family, nurses, physicians, area hospitals, or other Band staff accepted. Contact the nearest health centre for more information.
